  • Business Documentation such as Tax Id letter, Articles of Incorporation, Secretary of State Certification
  • Business Tax ID number (EIN)
  • Business physical address
  • Name of control person
  • Names of all business owners with at least 25% ownership
  • Opening deposit of $500
